Subcontractor Documents
Before we work with a new subcontractor, there are several compliance documents we need to collect. The goal of collecting these documents is to ensure that the subcontractor will complete the job as specified and to ensure that the subcontractor has the necessary insurance and bonds to cover any damages that might happen during the project. Please submit a copy of your Certificate of Liability Insurance (example) showing E.T. Technologies as additional insured

Please submit a copy of your Experience Modification Rating (EMR), also known as Mod Rating

OHSA 300/300a Logs
Please submit your OHSA 300/300a injury and illness logs for the last three years, if applicable. For more info: osha.gov/recordkeeping/forms
